Calf Pain Treatment Using LLLT/PBMT
Low Level Laser Therapy (LLLT), also known as Photobiomodulation Therapy (PBMT), uses red and near-infrared light applied over targeted areas. In our clinic it is used as part of a GP-led treatment approach for selected musculoskeletal pain and injury presentations, including selected calf muscle and tendon pain presentations.
PBMT/LLLT is non-invasive and generally well tolerated. Suitability and response vary, and treatment recommendations depend on the diagnosis, severity, duration of symptoms and individual clinical factors. - Calf pain is commonly caused by muscle strain, tendon irritation or overuse, often following physical activity, sport or sudden movements.
- It can also occur with poor warm-up, reduced flexibility, muscle imbalance, dehydration or unsuitable footwear.
- Typical symptoms include sharp or aching calf pain, stiffness, swelling, bruising or discomfort when walking, running or pushing off the foot.
- New, unexplained calf pain, especially with swelling, redness or warmth, requires medical assessment to exclude deep vein thrombosis (DVT).
- Low Level Laser Therapy (LLLT), or Photobiomodulation Therapy (PBMT), is considered for selected calf pain presentations where muscle or tendon irritation, pain or local inflammatory processes are contributing to symptoms.
- LLLT may be particularly relevant where pain limits rehabilitation or where loading, stretching or exercise aggravates sensitive tissues.

- LLLT can be considered for people with persistent calf pain or calf strain who have not improved with usual conservative care, after appropriate medical assessment.
What is Calf Pain?
Calf pain refers to discomfort or injury in the muscles at the back of the lower leg, particularly the gastrocnemius and soleus muscles. These muscles are important for walking, running, jumping and pushing off the foot.
Calf pain can occur due to muscle strain, tendon irritation, overuse or other injuries. It is common in athletes and in people undertaking activities that involve running, jumping or sudden stops and starts.
Causes of Calf Pain
Several factors may contribute to calf pain, including:
- Overuse or excessive strain during running, jumping or sudden acceleration
- Sudden increases in activity intensity, especially after a period of rest
- Inadequate warm-up before physical activity
- Muscle imbalance or reduced flexibility
- Dehydration or inadequate electrolyte balance
- Previous calf injuries that have not fully recovered
- Poor footwear or altered exercise technique
Although athletes are commonly affected, calf pain can occur in anyone following unusual loading, increased activity or prolonged deconditioning.
Important Consideration: Deep Vein Thrombosis (DVT)
Acute or new-onset calf pain that is not clearly related to injury may be a sign of deep vein thrombosis (DVT), a potentially serious condition that requires specific medical assessment and management. DVT may cause calf swelling, pain, redness or warmth and is not treated with PBMT/LLLT.
If DVT is suspected, urgent medical assessment and appropriate imaging, such as ultrasound, are required before considering any musculoskeletal treatment.
Symptoms
The symptoms of calf pain typically include:
- Sharp or aching pain in the calf during or after activity
- Stiffness or tightness in the calf muscles
- Swelling or bruising in the calf area
- Pain when walking, running, or stretching the leg
- Difficulty standing on tiptoe or pushing off the ground
- Weakness or reduced flexibility in the calf muscles
More severe muscle tears may cause bruising, swelling, a visible bulge or a palpable defect and require further assessment.
How is Calf Pain Diagnosed?
Diagnosis is based on a detailed clinical history and physical examination. Dr Shikha Parmar will assess the onset of pain, activity level, functional impact, tenderness, strength and movement.
Ultrasound or MRI may be used to evaluate muscle or tendon injury, rule out other causes, and determine whether there is a tear. X-rays are generally reserved for suspected bone or joint pathology.

Our Treatment Program
All patients are assessed by Dr Shikha Parmar (GP). Treatment is delivered within a structured six-week program, including:
- Up to 12 LLLT sessions
- Early progress review after the first 6 treatments
- Individualised advice and activity modification
- Guided rehabilitation to support longer-term function
Progress is reviewed during the program. Many patients notice appreciable improvement after the first 6 treatments. If there has not been sufficient improvement by this review point, further treatment may not be recommended.
